We do direct billing with most insurance companies.

Upon booking your appointment, you will receive three forms to be filled out. The first one is a Health History form; this form must be filled out by law. The third form is an optional Insurance Benefits form which gives us permission to directly bill your insurance provider if you so choose. 

Direct billing is a courtesy we offer clients of Carlington Massage Therapy Clinic. Most insurance companies/plans offer direct billing but not all. Please check with your insurance provider to ensure your plan offers this service. You will be responsible for paying for the treatment should the direct billing option not be available for your plan.

Appointments are reserved just for you. Please give 24 hours notice if you have to cancel. Cancellation with less than 24 hours will result in a full charge for the missed appointment.
